Digital Tools Could Improve Dementia Diagnosis in Primary Care Settings

Gerontological GSA and AAFP explain digital cognitive tests for Alzheimer/dementia, EHR-ready, repeatable, supports KAER care in primary care.

Early detection of cognitive decline, including Alzheimer disease and related dementias, remains a critical priority in clinical research. However, a recent report by the Gerontological Society of America (GSA) and the American Academy of Family Physicians (AAFP) reveals a significant gap between clinical interest and current practice. While timely detection is a "critical lever" for managing dementia, most clinicians still rely on analog, paper-based tools may lack the sensitivity to catch subtle markers of early-stage decline.1

“Current reliance on analog, paper-based screenings introduces a level of subjectivity and coarseness in data that may miss the subtle markers of early-stage decline,” said, Lisa C. McGuire, PhD, GSA vice president of Strategic Alliances and Practice Innovation.1

Surveys conducted in late 2025 show nearly 9 in 10 AAFP respondents (88%) and 77% of GSA respondents continue to use standardized paper assessments like the Montreal Cognitive Assessment or Mini-Mental State Examination. In contrast, only 3% of AAFP and 13% of GSA members currently utilize digital cognitive assessments (DCAs).1,2

Despite low current usage, there is overwhelming clinician interest in modernizing the diagnostic process. More than 95% of respondents in both groups reported being "very or somewhat interested" in adopting DCAs. These technology-based tools offer standardized, efficient approaches provide granular insights into cognitive trajectories. Clinicians identified the generation of quantifiable data to guide decision-making and the ability to enable earlier detection as the top clinical advantages of DCAs.

Transitioning to digital platforms is not without challenges. The primary hurdles cited include a lack of integration into electronic health records (EHRs), insufficient clinical guidelines, and patient difficulty with technology. Furthermore, while 79% of AAFP and 65% of GSA members are open to remote or at-home administration, concerns persist regarding test accuracy and reimbursement.

To accelerate adoption, the report identifies key priorities:

  • Seamless EHR incorporation
  • Consistent guidance on billing and coding
  • Targeted continuing medical education
