
Tirzepatide Plus Ixekizumab Improves Outcomes in Psoriatic Arthritis With Obesity: AAD 2026
Phase 3b findings suggest that combining a GLP-1/GIP receptor agonist with IL-17 inhibition may improve both joint and metabolic outcomes in adults with psoriatic arthritis and obesity.
At the American Academy of Dermatology (AAD) Annual Meeting, investigators reported that the combination of tirzepatide and ixekizumab demonstrated superior efficacy compared with ixekizumab alone in adults with psoriatic arthritis (PsA) and obesity, addressing 2 interrelated disease drivers in a high-risk population.1
"In TOGETHER-PsA, treating PsA and obesity concurrently with Taltz and Zepbound yielded meaningful, broad improvements in PsA disease activity, inflammation, and outcomes that can impact patients' daily lives, such as fatigue, disability and quality of life," said Philip Mease, M.D., Director of Rheumatology Research, Swedish Medical Center and Clinical Professor at the University of Washington, Seattle and TOGETHER-PsA study author. "These two chronic inflammatory diseases are often intertwined, with patients managing a substantial disease burden that remains difficult to treat. This clinical evidence supports a potential transformation in how we approach treatment for this patient population."2
The phase 3b study evaluated adults with PsA and obesity, a population known to have more severe disease, reduced treatment response, and higher cardiometabolic risk.
Participants were treated with:
- Ixekizumab (an IL-17 inhibitor) alone
- Ixekizumab plus tirzepatide (a dual GLP-1/GIP receptor agonist)
The rationale for combination therapy reflects emerging evidence that obesity contributes to systemic inflammation and may attenuate response to biologic therapy in PsA.
The combination regimen demonstrated superior efficacy across multiple domains, including:
- Greater improvements in joint symptoms and disease activity
- Enhanced weight reduction compared with biologic therapy alone
- Improved overall clinical response in patients with dual inflammatory and metabolic burden
These findings suggest that targeting both inflammatory pathways and metabolic dysfunction may provide additive or synergistic benefit.
Clinical Context for Primary Care
Psoriatic arthritis frequently coexists with obesity, and both conditions are commonly managed or co-managed in primary care settings. Obesity is associated with:
- Increased PsA severity
- Reduced response to biologic therapies
- Higher cardiovascular risk
For primary care clinicians, these data reinforce the importance of:
- Screening for PsA in patients with psoriasis and obesity
- Addressing weight management as part of inflammatory disease care
- Coordinating multidisciplinary treatment strategies
Mechanistic Rationale
The combination approach targets 2 complementary pathways:
- Ixekizumab reduces IL-17–mediated inflammation central to PsA pathogenesis
- Tirzepatide improves weight, insulin sensitivity, and systemic inflammation
By reducing adiposity and metabolic inflammation, tirzepatide may enhance responsiveness to biologic therapy.
The safety profile of the combination was consistent with known effects of each agent, with no unexpected safety signals reported in the study.
These findings support a paradigm shift toward integrated cardiometabolic and immunologic management in PsA.
